District Policy 5-303 covers student records and privacy issues.

The Family Educational Rights and Privacy Act (FERPA)

The Family Educational Rights and Privacy Act (FERPA), a Federal law, requires that Lake Havasu Unified School District #1, with certain exceptions, obtain your written consent prior to the disclosure of personally identifiable information from your child's education records. However, LHUSD may disclose appropriately designated "directory information" without written consent, unless you have advised the District to the contrary in accordance with District procedures.

The primary purpose of directory information is to allow LHUSD to include information from your child's education records in certain school publications and/or public media. Examples include:

• A playbill, showing your student's role in a drama production;

• The annual yearbook;

• Honor roll or other recognition lists;

• Graduation programs; and

• Sports activity sheets, such as for wrestling, showing weight and height of team members.

LHUSD may also publish, post or distribute a student’s name, photograph, audio and/or video recording, displays of student work or other information related to student achievement (ie: academic/athletic recognition or award) or student accomplishment (ie: a specially selected piece of work). Media includes, but is not limited to, newspaper, audio/video, live broadcast, internet, webpages, and social media.

LHUSD has designated the following information as directory information:

• Student's name

• Address

• Telephone listing

• Electronic mail address

• Photograph

• Date and place of birth

• Major field of study

• Dates of attendance

• Participation in officially recognized activities and sports

• Weight and height of members of athletic teams

• Degrees, honors, and awards received

• The most recent educational agency or institution attended

Student Directory Opt-Out

If you wish your child to be excluded from having his/her directory information released, the form is available through your school's front office.

Children Online Privacy Protection Act (COPPA)

In order for schools within the Lake Havasu Unified School District to continue to be able to provide your students with the most effective web-based tools, software, and applications for learning, our district utilizes several technology applications and web-based services, operated not by this district, but by third parties.

In order for our students to use these programs and services, certain personally identifying information must be provided to the web site operator.

COPPA Opt-Out

(for parents of school-aged children under the age of 13)

If you wish to exclude your student from utilizing and accessing third party technology applications and web-based services used in our schools, the form is available through your school's front office.

Technology Use Agreement

Lake Havasu Unified School District uses technology in all aspects of instruction which includes Internet use, email, computer, information technology (IT) devices, educational software, etc.  While the District will take all responsible precautions to protect your student, realize the District cannot prevent access to all controversial materials.  We expect your student to follow the District technology policy which is based upon federal, state, and local laws regarding acceptable use of computers and IT regardless of whether they belong to the District or to an individual.  Anyone who misuses, abuses, or chooses not to follow these guidelines and procedures will be denied access to the District's technology services and may be subject to disciplinary action.

The form must be signed by a parent and student and is part of the registration packet provided by your school.
